In-Game Description

    A latticed, deformed rock, or perhaps a meteorite.

    Appears useless, but possesses some old gravitational force
    that prevents its riddance. A dubious soul once said:

    "Step lightly round to the right of the great cathedral, and
    seek an ancient, shrouded church... The gift of the godhead
    will grant you strength..."

  • A familiar face with an unfamiliar body, Patches will make his presence known once you've entered the Forbidden Woods, from which point he will take every possible opportunity to force the Tonsil Stone upon you (he will take over various residents in and around Yharnam) and lure you into the clutches of his illegitimate god, Amygdala.

  • For more information about what residential NPCs (ones locked inside their homes) can be replaced by the Patches the Spider visit the Yharnam's Residents wiki page.

  • The first place you're likely to encounter him is at the house along the Village Outskirts near the kennels and the portcullis in the Forbidden Woods. Once you have the stone, return to the Cathedral Ward and go to the room at the end of the cliffs; approach the door at the other end of the room to trigger the False God's attack and let it hit you.

  • Having Tonsil Stone in your possession allows you to survive being grabbed by the False God at the entrance of Yahar'gul; its grab will instead teleport you to the first floor of the Lecture Building and you'll be able to access the Nightmare Frontier.

Availability

  • Acquired from (Main Story): Talk to any residential NPC after entering the Forbidden Woods, likely the first one to reach is in a house along the Village Outskirts in Forbidden Woods.

Notes

  • Any residential NPC that was sent into safe place [Iosefka's Clinic or Oedon Chapel] won't be replaced by Patches the Spider
  • If you didn't interact with any residents during the nighttime, Patches will replace only three residents during the Blood Moon: Young Girl & Older Sister; Male in Healing Church Workshop; Village Alleyways - Grounds Level and Female in Forbidden Woods Village Outskirts on path to Subterranean Cave Entrance. All other residents are not available and will have prompt "No response".
  • Upon starting New Game+ all of the Key Items (except for Badges, Blood Gem Workshop Tool, Rune Workshop Tool, Short Ritual Root Chalice, Workshop Haze Extractor and Yharnam Stone) you acquired on your previous playthrough will be stripped away and you'll need to collect them again.
